About the Position Job Title: Registered Nurse Case Manager

experience as a clinical nurse in a hospital is required - Nine
months of experience in home care is preferred License: - RN
License (NY License only) Certifications: - BLS - OASIS Must-Have:
- Certified home health care experience - EPIC or Netsmart
experience - Telemetry experience Description: Develops and revises
plans of treatment and discharge in conjunction with patients,
families, and care delivery teams. Completes and submits OASIS
assessments, reassessments, transfers, resumptions of care,
discharges, and significant changes in condition according to
agency-defined timeframes. Processes orders and notifies physicians
of patient needs and changes in condition. Provides an environment
that promotes respect for patients, their privacy, confidentiality,
and property. Initiates appropriate preventive and rehabilitative
strategies. Provides services requiring substantial specialized
nursing skills. Consults with and educates patients, families, and
other team members regarding disease processes, self-care
techniques, and preventive strategies. Supervises licensed
practical nurses to ensure patient health, safety, and compliance
with plans of care. Directs and evaluates the quality of patient
care services provided by the home health agency. Interacts with
patients, physicians, referral sources, and others to maintain
positive relationships. Supports care management philosophy,
focusing on collaborative, customer-focused planning and case
management to meet individual health and service needs. Ensures
compliance with state and federal legislation and smooth service
delivery within established timeframes. Maintains current knowledge
of available community-based services. Demonstrates flexibility and
willingness to visit patients based on need. Ensures problem
resolution at the point of contact. Ensures authorization and
release of information forms are signed by patients or responsible
parties. Attends patient care conferences on assigned patients.
Monitors results and takes corrective action on quality indicators,
including levels of customer satisfaction. Understands the
significance of the RN function concerning patient outcomes,
readmissions, productivity, utilization, revenue, and expenses.
Pursues efforts to eliminate avoidable costs and errors. Responds
to findings of quality management reviews and ensures resolution of
potential problem areas. Participates in peer review and quality
assessment and performance improvement activities. Maintains
productivity requirements. Ensures that appropriate visit
authorizations are in place and documentation reflects the medical
necessity for care. Promotes recognition, respect, and celebration
of workforce diversity. Participates in education and training
programs for professional growth and development. Demonstrates
commitment to professional growth and competency. Responsible for
key performance indicators established by management. Performs
