POSITION SUMMARY: Reporting to the Care Services Director, the Care
Services Coordinator is responsible for the management of care
services programs ensuring the uniform delivery of high quality,
consistent services for individuals with ALS and their families
within an assigned territory area. This position facilitates
education, information, benefit, and clinical resource navigation
assistance to persons diagnosed with ALS and caregivers. ESSENTIAL
DUTIES AND RESPONSIBILITIES: Utilize the Intake form to obtain all
necessary client, medical, demographic information; document to
support families in meeting goals. Conduct care consultations and
needs assessments for people living with ALS, caregivers, and
families in a variety of settings including virtual visits, by
phone/email, in clinic and support groups. Refers to Care Services
Manager for complex case-expertise area. Actively manage a
portfolio of clients, assuring client and caregiver needs are met
within the capabilities of The ALS Association. Provide information
on programs, services, resources and printed/electronic educational
materials available to persons with ALS and their families.
Coordinate referrals to ALS Association programs as well as
healthcare providers and community agencies. Facilitate resource
support group meetings, engage participants, and schedule speakers
as appropriate to meet the needs of the group. Document all
pertinent engagements with persons living with ALS, family,
caregivers, or other healthcare professionals involved with the
delivery of care and support in the database. Ensure quarterly
contact/engagement with those living with ALS and/or their
caregivers. Educate and provide those served on the available
nationwide, state, and local resources through utilization of The
ALS Association Resource & Referral Hub. Serve as a member of the
ALS Clinic multidisciplinary team and function as the liaison
between the clinic and The ALS Association. Participate in
continuing education seminars, workshops, and conferences as
available and necessary to gain knowledge on related topics.

QUALIFICATIONS: Bachelor’s degree in social work, nursing, public
health, or related area, preferred. Minimum of 3 years related
professional experience including direct care and/or case
management experience. Occupational therapist, physical therapist,
speech therapist or direct experience working with the ALS
population is a plus. Associate’s degree may be considered, if
combined with three or more years of experience in a similar role.
Ability to work independently with little supervision; must be a
self-starter. Knowledge of ALS preferred, but not required.
Knowledge of Medicare, Medicaid, patient assessments skills,
planning and case management preferred. Commitment to organization
and highly efficient and conscientious about thorough follow-up.
Skilled communicator, with superior interpersonal, verbal, and
written skills. Ability to handle confidential information and
maintain high confidentiality. Excellent organizational and writing
skills as well as diligent attention to detail. Employee must be
able to lift or move 10 pounds, infrequently lift and/or move up to
25 pounds. Proficient computer skills, especially with Microsoft
Office and other databases. Experience with Salesforce is a plus.
Ability to drive locally and travel nationally on Association
business is required or requested. Ability to work occasional
evenings and/or weekends for Association business and events. PAY
TRANSPARENCY: The ALS Association’s pay range for this position is
$22.51 - $27.99 per hour. The ALS Association has compensation
ranges for various work locations throughout the United States,
allowing us to compensate employees competitively and consistently
in diverse geographic markets. The above-listed listed range shows
prospective compensation for this role; the appropriate range will
be established by what is eventually determined to be the
candidate's primary work location. Various factors, including the
complexity and significance of the role, job duties/requirements,
relevant experience and abilities, and primary work location,
determine individual compensation. Compensation ranges are
evaluated and typically adjusted on a regular basis. Offers are
made within the compensation range that is in effect at the time of
the job offer. The Association may, at its discretion, offer
merit-based increases tied to individual performance. The benefits
