Position Job Title: Occupational Therapist Profession: Occupational
Therapist Specialty: Therapy Duration: 13 weeks Shift: Monday -
Friday Hours per Shift: 8:00 AM - 4:30 PM Experience: Minimum of
one year in a Long Term Care/Home Care setting License: Current
Occupational Therapy license to practice in New York State
Certifications: OASIS Must-Have: Recent home care experience within
the last year Description: Function: Provides case management and
direct patient care within a team environment. Administers
comprehensive skilled care. Participates and assures that care
delivery services are consistent with the agency vision, mission,
and values. Promotes effective working relationships and works
effectively as part of the team to meet organizational goals.
Utilizes resources and supplies efficiently to contain costs and
increase service value. Promotes positive communication to all
external sources, patients, families, and staff. Displays behaviors
that support the overall organization. Supports public relations
and community marketing initiatives. Strives for continuous
improvement in all aspects of occupational therapy functions.
Develops and maintains a customer-oriented environment responsive
to the needs of all customers. Direct Patient Care Duties: Develops
and revises treatment plans and discharge plans in conjunction with
patients, families, and care delivery teams. Provides an
environment that promotes respect for patients and their
confidentiality. Initiates appropriate preventive and
rehabilitative strategies. Educates patients and families regarding
disease processes and self-care techniques. Supervises support
personnel to ensure patient health, safety, and compliance with
treatment plans. Coordinates, evaluates, and supervises quality
patient care services. Maintains positive relationships with
patients, physicians, and referral sources. Ensures the completion
of all appropriate clinical records for compliance. Refers to
community resources as needed. Maintains current knowledge of
available services. Demonstrates flexibility and willingness to
cooperate with customers and team members. Ensures problem
resolution at the point of contact. Makes constructive
recommendations for improving quality of care and customer service.
Sets an example of integrity. Participates in identifying potential
new programs and services. Ensures patient needs are met through
the care delivery process. Ensures authorization and release forms
are signed by appropriate parties. Ensures compliance with agency
performance standards. Participates in achieving operational and
financial objectives. Monitors quality indicators and takes
corrective action as needed. Understands the impact of occupational
therapy on patient outcomes and operational efficiency. Pursues
efforts to reduce or eliminate avoidable costs and errors. Develops
quality plans of care consistent with guidelines. Responds to
findings of quality management processes and ensures resolution of
