Position Job Title: Physical Therapist Profession: Physical
Therapist Specialty: Occupational Therapist Duration: 13 weeks
Shift: 5x8.0 Hours per Shift: Monday - Friday 8:00 AM - 4:30 PM;
weekends as needed Experience: Minimum of one year experience in
Long Term Care/Home Care setting as Physical Therapist. License:
Current New York State License in Physical Therapy. Certifications:
BLS (AHA ONLY) Must-Have: Oasis experience Description: Provides
case management and direct patient care within a team environment.
Administers comprehensive physical therapy modalities in the home
setting. Assists the physician in evaluating level of function and
helps develop and revise the plan of care as necessary. Prepares
clinical and progress notes and advises and consults with the
family and other personnel. Participates in in-service programs.
Participates and assures that care delivery services are consistent
with the agency vision, mission, and values. Promotes effective
working relationships and works as part of the team to meet goals
and objectives. Utilizes resources and supplies efficiently to
contain costs and increase the value of services. Promotes positive
and respectful communication to all patients, families, and staff.
Strives for continuous improvement in all aspects of the physical
therapist function. Develops and revises plans of treatment and
discharge in conjunction with patients, families, and the care
delivery team. Initiates appropriate rehabilitative programs and
provides supervision of assistants to ensure patient health and
compliance. Consults with and educates patients and families
regarding disease processes and preventive strategies. Implements
home therapy programs, including gait training, transfers, and
activities of daily living. Periodically evaluates patient progress
and makes necessary adjustments to the physical therapy program.
Directs, coordinates, and supervises the quality of patient care
services provided by the home health agency. Maintains
relationships with patients, physicians, and referral sources in a
positive manner. Supports care management philosophy and
collaboratively develops customer-focused planning. Participates in
regularly scheduled patient conferences for the purpose of
discussing care and patient needs. Assists in the orientation of
new rehabilitation personnel. Ensures that all appropriate clinical
records are completed for compliance with regulations. Maintains
current knowledge of available services and demonstrates
flexibility and enthusiasm while working with others. Ensures
resolution of problems at the point of contact and makes
constructive recommendations for quality improvement. Maintains
productivity requirements and monitors results in relation to
customer satisfaction. Contributes to an environment of
professional growth and mutual respect for all employees.
Participates in education and training programs, demonstrating
commitment to professional growth. Responsible for achieving key
